Chronic Care management is a program offered by Medicare for patients with two or more chronic health conditions to help improve quality of life and quality of care. This program allows the care coordinator to work with your doctors and care team to set goals and manage your health conditions to avoid unwanted hospital or emergency room visits. The Preventative Care Program will discuss the importance of preventative screening and discuss how our community can lower their risk for higher level health complications (cancer, hypertension, etc).

Hammond-Henry Hospital and Virgil Thurman, Attorney at Law, will present on what things to communicate to your family to assure that your wishes are carried out in times of emergency. Virgil will speak to steps that are necessary to produce a living will. We will also discuss how that communication is handled in the hospital to assure that our caretakers meet your wishes.

Hammond-Henry will team up with local hospice teams to provide information on what hospice is, why it is important in end of life care, when it is appropriate, and how care plans change when hospice is involved. Many have a negative and incorrect understanding of hospice and we look forward to breaking down those misunderstandings for our community members.

This session will go through the biomechanics of the golf swing and provide examples of golf warm up ideas to help improve mobility in the neck, spine, shoulders, hips, knees, and ankles specific to the golf swing.
